Through the Eyes of Healthcare Providers, the Necessity and Challenges of Implementing Medication Reconciliation at Hospital Discharge: A Qualitative Study

Background: Medication reconciliation can include medication reviewing and providing counseling and a list of all the medications during every transition of care. Objectives: to explore in-depth the perspectives of Iraqi physicians and pharmacists regarding the necessity of medication reconciliation at hospital discharge and identify the possible benefits and challenges that could face its implementation. Subjects and Methods: A qualitative study included semi-structured interviews with pharmacists and physicians working at a public teaching hospital in Iraq. The interviews were conducted face-to-face from February to March 2023. Thematic analysis was used to analyze the qualitative data generated from the interviews. Results: In this study, 18 healthcare providers were recruited: 12 pharmacists and six physicians. All participants supported medication reconciliation adoption at hospital discharge. Pharmacists can identify unintended medications discrepancies via discharge medication reviewing, patient counseling and providing discharge list. The most frequently reported challenges to establishing medication reconciliation were a lack of clear official instructions, suboptimal physician-pharmacist cooperation, and inadequate awareness. Conclusions: The implantation of medication reconciliation has numerous benefits in terms of enhancing patient safety and avoiding medication errors. However, it needs several pre-requisites to be implemented routinely, including official enforcement and training courses to raise healthcare providers’ awareness.

Spectroscopic Study of Plasma Parameters Produced by Pin-Plate DC Discharge Technique

     In this work, plasma parameters such as electron density (ne), electron temperature (Te), Debye length (λD), plasma frequency (fPlasma), and Debye number (ND) for Cu plasma produced by Pin-Plate DC discharge were studied. Spectroscopic technique was used to analyze and determine spectral emission lines. The value of the electron density for Cu was in the range (1.5–3.5)×1018cm-3 and for the electron temperature was in the range ( 1.31 – 1.61)eV. Finally, plasma parameters of Cu were caculated through plasma produced by Pin-Plate DC discharge using different voltages (600-900) V.

A Qualitative Study of Epiphytic Algae (Diatom) on Some Aquatic Plants in Al-Auda Marshes Within Maysan Province / Southern Iraq

The study conducted on the compositions of epiphytic diatoms on three taxa of aquatic plants were selected (Phragmites australis Trin ex stand , Ceratophyllum demersum L. and Typha domengensis Pers) in three sites within Al-Auda Marsh, from autumn 2013 to summer 2014 . The study was measured physical and chemical factors of all the study sites, such as: air temperature, power of hydrogen (pH), electrical conductivity (EC), salinity (S‰), total hardness(TH), dissolved oxygen (DO), and plant nutrient. Current and Modified Flood Discharge Capacity of a Reach of Tigris River between Kut and Amarah Barrages

This study was conducted to examine the discharge capacity of the reach of the Tigris River between Kut and Amarah Barrages of 250km in length. The examination includes simulation the current capacity of the reach by using HEC-RAS model. 247cross sections surveyed in 2012 were used in the simulation. The model was calibrated using observed discharges of 533, 800, 1025 and 3000m3/s discharged at Kut Barrage during 2013, 1995, 1995 and 1988, respectively, and its related water level at three gauge stations located along the reach.
