The aim of this study is to design a proposed model for a document to insure the mistakes of the medical profession in estimating the compensation for medical errors. The medical profession is an honest profession aimed primarily at serving human and human beings. In this case, the doctor may be subject to error and error , And the research has adopted the descriptive approach and the research reached several conclusions, the most prominent of which is no one to bear the responsibility of medical error, although the responsibility shared and the doctor contributes to them, doctors do not deal with patients according to their educational level and cultural and there are some doctors do not inform patients The absence of a document to insure the medical error and educate the medical side of the importance of the document, providing medical errors provides a stable environment for the doctor.
The Hartha Formation reservoir was evaluated to be produced in the near future in Y oil and J fields, West of the Tigris River in northern Iraq. Due to geological data analysis of the Hartha facies and diagenesis could enhance oil production with minimized risks, this research tried to deal with data to support the oil production. The Hartha reservoir is subdivided into five-rock units, Har-1, 2, 3, 4 and 5. It is of Heterogenic facies limestone, dolomite, dolomitic limestone and argillaceous dolomite. The study aimed to evaluate whether Enalapril has any effects on the testicular tissue of the albino male mice which received or did not receive treatment with Enalapril.
Mature mice were divided into three groups: control group which fed with 0.1mg of normal saline, the second group was orally fed with 10 mg/Kg of Enalapril drug while the third group was fed with 20 mg/Kg of drug, one time a day for 30 successive days by using gavage tube (0.1ml). The histological sections were performed and the slides were examined using the light microscope.

The Sr doped La1Ba1-xSrx Ca2Cu4O8.5+δ samples with 0 ≤ x ≤ 0.3 had been prepared using the solid state reaction. The samples were claimed at 800°C for 3hr, palletized and sintered at 860°C for 20hr in air . Dielectric constant and loss by means of capacitance have been investigated with frequencies in the range of 1kHZ to 1MHZ for our samples at room temperature. Also, Shore hardness has been measured. The dielectric constant and loss decrease slightly with the increase of frequency for all compounds.
