Acute decompensated heart failure (ADHF) is a leading cause of hospital admission and many factors are known to precipitate decompensation. We aimed to assess the decompensating factors of heart failure and the management of patients admitted to the emergency department (ED). A total of 107 patients were examined, all diagnosed with ADHF in the ED of the Baghdad Teaching Hospital, from June 2017 to December 2017, and presenting with decom¬pensation (pulmonary oedema, peripheral oedema, and fatigue).      Fifty patients(24 female and 26 male)with pressure ulcersassociated with different diseasesand attending AL-yarmouk Teaching Hospital in Baghdad were selected in this study. The duration of sample collection was from March  to December 2018. All blood samples collected from patients were submitted to a blood culturing technique to examine bacteremia. The results showed that12 blood bacterial isolates were obtained. The isolated bacteria were subjected to Vitek-2, which is an accurate identification technique. The results of the blood culturing technique revealed that 33.3% were Gram negative bacteria, while 66.6% were Gram positive. From different hospitals in Baghdad city, 25 clinical isolates of Proteus spp. were collected from different clinical samples, all isolates were identified as Proteus mirabilis by using bacteriological and biochemical assays in addition to Vitek-2 identification system. 15 (60%) isolates were identifying as Proteus mirabilis.
