The Surgical outcomes of fundus-first technique in lowering the rate of bile duct injuries and bleeding during open cholecystectomy that facing intraoperative difficulties: A single-center prospective study: The surgical outcomes of open fundus-first cholecystectomy technique.

Background: The problem of difficult gallbladder is not clearly defined and associated with real missing of therapeutic approaches that decreased morbidity. Moreover, the difficult gallbladder was reported as a contributing risk factor for biliary injury due to raised difficulty in surgical dissection within Calot’s triangle. The aim of this study is to determine the surgical outcomes of the open fundus-first cholecystectomy in lowering the rate of lethal intraoperative risks.

Subjects and Methods: Our prospective study conducted during the period of January 2019 to December 2022 at Ibn Sina specialized hospital, Khartoum, Sudan, for two hundred and fifty-three patients underwent elective open fundus-first cholecystectomy for intraoperative difficulties.

Results: the majority of cases had long-standing cholelithiasis with intraoperative difficulties revealed by pre-operative TUS and MRCP which required open fundus-first cholecystectomy in 173 (68.4%) of patients, (P-value <0.05). The operative surgical time was 120 minutes in 103 patients (40.7%), (P-value <0.05). During open cholecystectomy, variable intraoperative difficulties were found in all patients, (P-value <0.05) and were divided into: (A) Contracted intra-hepatic gallbladder seen in 157 (62.1%) of cases; (B) Distorted anatomy within Calot's triangle seen in 135 (53.4%) of patients; and (C) Intraoperative bleeding from the liver bed was observed as bloody oozing form in 150 (59.3%) of cases. Our patients had minimal postoperative complications like mild wound infection in 8 (3.2%) of cases and mortality rate was zero.

Conclusions: There is a need for appropriate therapeutic and preventive strategies in healthcare systems for safe dealing with difficult cholecystectomy. Unclear anatomy due to severe inflammatory dense adhesions at Calot’s triangle and CBD stones needing difficult surgeries are the most important limiting factors for fundus-first laparoscopic cholecystectomy. Hence, we humbly recommend an open approach of fundus-first cholecystectomy as safe surgical option to reduce the incidence of bile duct injuries and intra-operative bleeding.

Calculation of the Best Stability Orbit of the Satellite around the Earth before Transferring to Orbit around Mars

In this research, the eccentricity will be calculated as well as the best height of satellite orbit that can used to transfer from that orbit around the Earth to construct an interplanetary trajectory, for example Mars, when the transfer can be accomplished by a simple impulse, that means the transfer consists of an elliptical orbit from the inner orbit (at a perigee point) to the outer orbit (at apogee point). We will determine Keplerian equation to find the value of a mean anomaly(M) by Rung-Cutta method.

There are several types of satellites orbits around the Earth, but by this study, we find that the best stable orbit to the satellite that is used to inter its orbit around Mars is the Medium Earth Orbit (MEO) at a hei

Study the Behavior of Castellated Steel Column Encasing by Different Reactive Powder Concrete Thickness with Laced Reinforcement

Castellated columns are structural members that are created by breaking a rolled column along the center-line by flame after that rejoining the equivalent halves by welding such that for better structural strength against axial loading, the total column depth is increased by around 50 percent. The implementation of these institutional members will also contribute to significant economies of material value. The main objectives of this study are to study the enhancement of the load-carrying capacity of castellated columns with encasement of the columns by Reactive Powder Concrete (RPC) and lacing reinforcement, and serviceability of the confined castellated columns. The Castellated columns with RPC and Lacing Reinforcement improve com

Study of the Electric Quadrupole Moments for some Scandium Isotopes Using Shell Model Calculations with Different Interactions

The electric quadrupole moments for some scandium isotopes (41, 43, 44, 45, 46, 47Sc) have been calculated using the shell model in the proton-neutron formalism. Excitations out of major shell model space were taken into account through a microscopic theory which is called core polarization effectives. The set of effective charges adopted in the theoretical calculations emerging about the core polarization effect. NushellX@MSU code was used to calculate one body density matrix (OBDM). The simple harmonic oscillator potential has been used to generate the single particle matrix elements. Our theoretical calculations for the quadrupole moments used the two types of effective interactions to obtain the best interaction compared with the exp

Numerical Study of Radiative Magnetohydrodynamics Viscous Nanofluid Due to Convective Stretching Sheet with the Chemical Reaction Effect

A numerical investigation was performed for the radiative magnetohydrodynamic (MHD) viscous nanofluid due to convective stretching sheet. Heat and mass transfer were investigated in terms of viscous dissipations, thermal radiation and chemical reaction. The governing Partial Differential Equations (PDEs) were transformed into an arrangement of non-linear Ordinary Differential Equations (ODEs) by using the similarity transformation. The resulting system of ODEs is solved numerically by using shooting method along with Adams-Moulton Method of order four with the help of the computational software FORTAN. Furthermore, we compared our results with the existing results for especial cases. which are in an excellent agreement. The
