Gentamycin injection in disc space during lumbar discectomy (aiming to prevent post - operative discitis)

Background: Postoperative discitis is uncommon 4%, but it is the most disabling postoperative complication after lumbar discectomy.
Objective: To assess the effectiveness of intradiscal Gentamycin in prevention of lumbar discitis post operatively.
Patients and Methods: This is a prospective study involving 320 patients, who had lumbar discectomy. 140 patients (first group) from Jan. 2012 to Jan. 2013 were not had intradiscal gentamycin injection, while 180 patients (2nd group) From Jan 2013 to Jan 2014 (2nd group) had intradiscal injection of 80mg Gentamycin at the end of surgery and before closing up. Follow up was done 10 days to 6weeks after surgery by W.B.C count, ESR, C-reactive protein and clinical assessment.
Results: There were eight cases (6%) of postoperative discitis in the first group, while in the second group there was only one patient (0.5%) with postoperative discitis who had intradiscal injection of Gentamycin (0.5%).
Conclusions: Intradiscal Gentamycin injection during lumbar discectomy is effective in preventing of postoperative discitis.

Irrigtion of Gallbladder bed with bupivacaine for post- operative pain relief in laparoscopic cholecystectomy.

Background: Although pain after laparoscopic cholecystectomy is less intense than after open Cholecystectomy, some patients still experience considerable discomfort, and use of local anaesthetics irrigation is controversial.

Objectives :to evaluate the effect of intraperitoneal and port site instillation of local anaesthetics on pain relief in the first 6 hours postoperatively.

Patient and Method: Forty patients underwent elective laparoscopic cholecystectomy were included in this study and sample was divided into two equal groups. Group A received 20 ml of 0.9% normal saline instilled in the gallbladder bed, while group B received 20 ml of 0.25% bupivacaine instilled in

Post-Operative Computed Tomography to Evaluate the Accuracy of Thoracic and Lumbosacral Spine Pedicular Screw Fixation

Background: pedicular screw fixation is rapidly becoming a widely used method of spinal instrumentation. Despite improvement in design of instruments and surgical technique and the use of intra-operative fluoroscopy, pedicle cortical perforations occur endangering nearby neurovascular or extra vertebral structures.
Objective: This is a prospective (consecutive cases study) designed for Evaluation of incidence of pedicle screw misplacement in our study sample, compare our results to what is published in literature.
Patients and Method: This study involved 25 adult patients ( 10 males and 15 females ) their age ranged between ( 18 and 70 years) who underwent spinal surgery with pedicle screw fixation for different disorders of

Background: Articular cartilage lesions in the knee joint are frequently seen, its treatment represents a challenge for orthopedic surgeons especially in young age patients and if left untreated, then they can progress to advanced osteoarthritis that may affect the functional status of the patients. Many treatment modalities had developed for these lesions, in the current study arthroscopic micro fracture plus intra articular whole bone marrow(WBM) injection was used as modality of treating these lesions.
Objectives: to assess the effectiveness of arthroscopic micro fracture plus WBM intra articular injection as a treatment modality for knee articular cartilage lesions.
Patients and Methods : a prospective experimental study was perfo

Proposed Methods To Prevent SQL Injection

In the last decade, the web has rapidly become an attractive platform, and an indispensable part of our lives. Unfortunately, as our dependency on the web increases so programmers focus more on functionality and appearance than security, has resulted in the interest of attackers in exploiting serious security problems that target web applications and web-based information systems e.g. through an SQL injection attack.     SQL injection in simple terms, is the process of passing SQL code into interactive web applications that employ database services such applications accept user input  such as form  and then include this input in database requests, typically SQL statements in a way that was not intende

The Monosynaptic H-reflex excitability in post-Stroke patients.

Backgrond: One of the major causes of impaired movements post-stroke is the abnormal phasing of paralytic muscles. Many studies suggested that inappropriate muscle phasing may be associated with enhanced transmission in the monosynaptic Group Ia afferent pathway in the affected limb of post-stroke patients and Group Ia reflexes are abnormally elevated and fail to decrease in amplitude during locomotion.

Objectives: This study was conducted to identify the changes in the Soleus muscle H-reflex excitability at rest in the affected lower limb of post-stroke patients as compared to the contra lateral side and of normal controls.

Patients and methods: The excitability of the m
