Objective : The study was carried out to construct an initial assessment documentation tool for nursing
recording system in Coronary Care Unit.
Methodology : A descriptive, purposive sample of (65) nurses was selected from CCU of main
teaching hospitals (Al Karama, Al Kindy, Al Kadimia, Al Yarmmok, Baghdad teaching hospital, Ibn
Al Naffis hospital) and Ibn-Al betar hospital in Baghdad city from the 15th of April 2004 to the 15th of
April 2006.
The instrument was constructed and comprised of two sections: section one included the
nurses' demographic characteristic; section two was the initial assessment documentation tool that
contained (2) parts including: General information form and the initial assessment form.

Hydatidosis is a zoonotic, helminthes parasitic disease that cause veterinary and human problems due to economic burden. Amis of the current study was to estimate the prevalence rate of hydatidosis infecting different organs of sheep slaughtered in street. Motivation is one of the most important factors that influence students learning and has a great effect on their success and achievement. For this reason, in this paper, the researcher is going to investigate the motivational teaching strategies used by EFL Iraqi school teachers. To achieve the aim of the study, the researcher adapted a questionnaire with some changes and modification’s. This questionnaire with five point scale (always, often, sometimes, rarely and never). To achieve face validity, the questionnaire items are given to a jury of experts. Alpha Cronbach formula is used to achieve the questionnaire reliability.
