Vibriosis-Associated Acute Kidney Injury: Incidence and Outcome

Abstract<p><bold>Purpose: </bold>Vibriosis, caused by <italic>Vibrio cholerae</italic>, is an intestinal infection characterized by profuse watery diarrhea, leading to significant fluid loss. This study aimed to assess the clinical features, outcomes, and severity of cholera among hospitalized patients and its association with acute kidney injury (AKI). <bold>Methods</bold>: A prospective cohort study was conducted on 70 patients with confirmed <italic>V. cholerae</italic> infection at Baghdad Teaching Hospital, Medical City, during the cholera outbreak in Iraq (June–October 2022). Patients were recruited from the emergency department and medical quarantine ward. Diagnosis was based on clinical features and confirmed by stool culture using Thiosulfate-Citrate-Bile Salts-Sucrose (TCBS) agar, according to WHO criteria. <bold>Results</bold>: AKI was identified in 83% of patients. While most were managed conservatively, 13% required hemodialysis. The <italic>V. cholerae</italic> Ogawa serotype was isolated in 55.7% of cases. Patients with AKI were significantly older than those without. Abnormal skin turgor was noted in 82.8% of AKI cases and was significantly associated with kidney injury. Blood pressure did not differ significantly between groups. Severe dehydration was more prevalent among AKI patients. <bold>Conclusion</bold>: AKI is a common complication of cholera and may occur even in the absence of hypovolemic shock or severe dehydration. A notable proportion required renal replacement therapy, underscoring the need for early recognition and management of renal involvement in cholera<bold>.</bold></p>

Penetrating Cardiac Injury in a Child Following Nail Gun Accident: Clinical Implications and Public Health Concerns

Penetrating cardiac injuries caused by nail guns are exceedingly rare but often life-threatening, with reports showing an increasing trend. We described the case of an adolescent male who sustained accidental cardiac penetration by an iron nail while performing carpentry work. Rapid referral to a specialized cardiac center enabled timely surgical intervention, illustrating the pivotal role of early recognition, expedited transfer, and expert management in optimizing outcomes. This case also highlights the grave risks associated with the employment of minors in hazardous occupational settings, where exposure to unsafe environments may lead to catastrophic consequences.

Incidence of Ciprofloxacin-Resistant of Methicillin Resistant Staphylococcus aureus isolated from Iraqi patients

The resistance of Staphylococcus aureus to ciprofloxacin has complicated the problem of treating staphylococcal associated infections in which MRSA is the causative agent since ciprofloxacin was the drug of choice to treat such infections. Our study investigated the incidence of Ciprofloxacin resistant S. aureus isolates that were also methicillin resistant among Iraqi patients. The obtained bacterial isolates were tested for Ciprofloxacin resistance using agar dilution method and the sequence of gyrA and parC. The results revealed that about 8% of the isolated MRSA strains were Ciprofloxacin resistant and the resistance was due to mutation in gyrA rather than parC.

In present study, the snakes were collected from the city of Baghdad and transferred to the laboratory where their kidneys were dissected out. The samples were then processed to be prepared for histological examination microscopial observations showed that there is no border between the cortex and medulla regions of kidney. The kidney consists of nephrons which are composed of glomerulus surrounded by Bowman’s capsule; the other segments are proximal tubule, distal tubule and connecting tubule.
