خلفية البحث:  إن جراحة تثبيت العمود الفقري باستخدام مسمار قدم الفقرات أصبحت من الوسائل الشائعة في جراحه العمود الفقري. رغم التطور الذي في تقنيات و تصاميم وسائل هذه ألجراحه و استخدام التصوير ألشعاعي إثناء هذه ألجراحه لازالت هنالك احتماليه وضع هذا المسمار في غير موضعه الصحيح محدثا ثقوبا في قشره قدم الفقرات مهددا أصابه الأحشاء العصبية و الوعائية ما حول الفقرات.

Methods:
A total of 1450 pediatric cases with poisoning admitted to the Central Teaching Hospital of Pediatrics, Baghdad, during the 10 years study period extending from the 1st of
January 1993 to 31st of December 2002, were analyzed.
Results:
The peak age for poisoning cases in the present study was 1-4 years, constituting about three quarters of total pediatric admissions with poisoning.

Background: The alteration of bowel habits, bleeding per-rectum and anemia were common features in both groups in this study, but in young patients there was a delay of 6 months between the presenting symptoms and the definitive diagnosis because the disease was not suspected and investigated in them. The most common site for the tumors in young patients was the rectum and in patients above the age of 40 years was the Sigmoid.

Case Report: Common bile duct perforation has been reported in adults after invasive procedures, spontaneous common bile duct perforation is a rare entity as a cute abdomen in adults. A few cases due to choledocholithiasis have been reported as a cause of spontaneous perforation. We report an adult patient who presented with acute abdomen after spontaneous common bile duct perforation due to unknown etiology who was treated successfully.
